Disease Management Program

We are all aware of the large increases in medical costs related to obesity, diabetes, asthma, coronary artery disease. In most medical plans, 6-8% of the covered individuals consume over 65% of the plans total medical costs. Studies have shown that for each dollar invested in a comprehensive disease management program there is a cost savings of 20%. PCM’s Disease Management Program is directed to individuals who are identified as having a high risk factor. The program rewards individuals that achieve specific health related goals, Similar to a frequent flyer program, the closer the individual gets to their health goal, the more points they earn.

In order for any program to be successful, the key ingredient is individual participation. The second most important is motivating the individual to continue in the program over time to achieve their individual health goal. PCM's program addresses both of these issues.
